Cardioembolic Stroke secondary to Massive Stress-Induced Apical Thrombosis: A Clinical Conundrum relating to Anticoagulation Initiation

چکیده
Takotsubo cardiomyopathy or stress is a condition characterized by acute and transient left ventricular systolic dysfunction in the absence of coronary heart disease, occurring after an emotional physical stressful event. Cardiac these patients suspected to be secondary catecholamine induced cardiac myocyte injury via cyclic AMP-mediated calcium overload due endothelial dysfunction. Even though takotsubo transient, it can lead complications. Left thrombus formation widely reported complication has incidence around 5-14% thromboembolic events like stroke. We report case with apical LV thrombus, complicated large cardioembolic This constitutes clinical conundrum, as would warrant prompt initiation anticoagulation, while severe ischemic stroke contraindication for immediate anticoagulation.

Anticoagulation after cardioembolic stroke: to bridge or not to bridge?
BACKGROUND Most patients with cardioembolic stroke require long-term anticoagulation. Still, uncertainty exists regarding the best mode of starting long-term anticoagulation. Design, Setting, and Patients We conducted a retrospective review of all patients with cardioembolic stroke admitted to our center from April 1, 2004, to June 30, 2006, and not treated with tissue plasminogen activator. Pa...
متن کاملAcute anticoagulation following cardioembolic stroke.
Whether acute anticoagulation after cardioembolic stroke affords substantial protection against early recurrent emboli or an unacceptable risk of hemorrhage remains controversial. To assess this further, we evaluated 121 consecutive patients with acute cardioembolic stroke. Forty-nine were therapeutically anticoagulated within 96 hours of stroke onset, and 41 received no anticoagulants within t...
متن کاملClinical benefit of early anticoagulation in cardioembolic stroke.
BACKGROUND Nonvalvular atrial fibrillation is the most common source of cardiac embolism with a high reported risk of stroke and a high stroke-related mortality. A common clinical dilemma in patients with acute stroke is whether the detection of one of the major cardiac sources of emboli requires an early anticoagulation to reduce early stroke recurrence and mortality. METHODS In this review,...
متن کاملHypereosinophilic Cardiac Involvement Presenting With Left Ventricular Massive Thrombus and Cardioembolic Stroke: A Case Report
Introduction: It is well known that the tendency toward thrombosis is increased in cancer patients. The increase in cancer procoagulant and tissue factor levels, endothelial damage, and stasis due to compression are among the most accused causes of thrombosis in cancer patients. Hypereosinophilia is a rare condition that causes endothelial damage leading to thrombosis. <s...
متن کاملIntensity of anticoagulation and clinical outcomes in acute cardioembolic stroke: the Fukuoka Stroke Registry.
BACKGROUND AND PURPOSE The relationship between the intensity of anticoagulation at the onset of acute cardioembolic stroke and clinical outcome after stroke is unclear. Here, we elucidated the relationship between prothrombin time-international normalized ratio (PT-INR) values on admission and clinical outcomes in patients with acute cardioembolic stroke. METHODS A total of 602 patients from...
